Athletes entering grades 5-12

  • Athletes entering grades 5-12 this coming year are required to have a physical before practicing or playing any sport. Physicals completed prior to May 1, 2025 are not valid for the 2025-26 school year. 
  • Except for the back page, download and complete the entire physical packet and bring it to your appointment. The last page in the packet will be completed by a clinician.
